Referral Methods: Residents are self-referred for services.
Occasionally a friend or family member requests service coordinator
services for a resident and permission from the resident may then be
obtained if the resident agrees.
Eligibility
Criteria:
Site Location: Dayspring Apartments in Corning, NY
Services
Provided: At
the request of the residents, and with the permission of each resident
served, the Service Coordinator works with the resident confidentially
to identify unmet needs and then provides information, linkage, and/or
access to services that meet
the resident’s need(s) with the goal of attaining or maintaining a
good quality of life.
Hours
of Operation: 9:00am – 4:00pm, Monday - Friday
Cost
for Participant: There is no cost for Dayspring residents

by: This service is provided under the auspices of the Dayspring
Board of Directors with grant funding from HUD.
